Unsatisfactory Emails - need a comment section

When locations submit unsatisfactory ratings from emails, it does not give an open to submit notes or leave a description.  I would like to see this feature added so we have more information as to why the work performed was unsatisfactory. 

Once you click "excellent", "satisfactory" or "Unsatisfactory" it just sends the information and doesn't allow any additional feedback in the email response.
